This clinical scenario can be caused by numerous factors: the result of nerve damage, such as pudendal, caused by cycling, various pathologies (such as endometriosis) or derived from a previous surgery. Other times the origin can be myofascial, in the pelvic floor area. It is often a complex phenomenon, as it ends up involving problems related to sedestation, defecation, sexuality… This circumstance makes the treatment of pelvic pain very challenging.

Most Common Syndromes

In the presence of pelvic pain, having been excluded visceral origin for the pain, other causes should be kept in mind.

Fibromyalgia

Fibromyalgia is a pathologic condition where pain conducting systems are transmitting information in an erroneous manner. Hence, even in the absence of a physical injury that would cause pain, the person can have this pelvic pain. In approaching this type of pain, it is important to understand that it is a benign condition and, although it is not to be depreciated, the patient should be reassured. Physical exercise, medication and physiotherapy may help reduce the pain.

Pelvic floor muscle tension

Pelvic floor muscle tension is caused by an inability to properly control these muscles, leading to a continuous contraction and tension. Treatment includes medication, physiotherapy and occasionally local injections.

Inflammation of the pubic joint (pubic symphysis)

Pubic symphysitis describes an inflammation of the pubic symphysis joint, causing pelvic and/or perineal pain that is usually aggravated by bringing both legs against each other. Treatment involves resting, medication, physiotherapy and occasionally a steroid injection.

Chronic pelvic inflammatory disease

Pelvic inflammatory disease Is characterized by a superior reproductive system infection. The main symptoms are pelvic or lower abdomen pain, accompanied by fever, vaginal discharge or burning sensation while urinating. Treatment includes an antibiotic course and medication to help with symptoms.
